I am requesting to bind temporary coverage based on the information provided, which I confirm is accurate and complete. I understand a 30-Day Livestock Temporary Binder will be issued and emailed to me. Until Bascule Underwriting receives and approves a completed application, agreed values, and satisfactory health documentation, this binder insures only against death or humane destruction by accident from the perils of fire, lightning, and transportation — death from sickness, disease, or any other cause is at my risk. The binder is void after policy issue, automatically expires 30 days after the effective date, and may be cancelled by notice. I will notify my agent immediately of any illness, disability, or death of the animal.
